Failure Investigations
“Failures” take many forms and have a wide range of consequences which include:
- risk of injury to personnel
- damage to capital or rental equipment, or the environment,
- loss of well or process integrity.
The common “traditional” failures that receive attention are those that result in clear “Non-Productive Time” (NPT) because they are clearly visible and have an easily calculated financial cost. However, it is often the investigation of the less obvious failures
For example, a premature BHA failure due to vibration or insufficient hydraulics to clean the hole, due to incorrect drillpipe selection, that may actually yield more savings.
Events like these which are due to inefficient operations are more commonly classed as “Invisible Lost Time” (ILT).
Fearnley Procter have decades of experience performing many types of Failure Investigations in both NPT and ILT categories and this has resulted in the development of detailed procedures, software and protocols that can be used - these maximise the lessons that can be learned.
The first task is to preserve the evidence and capture all of the possibly associated data. You can never capture too much data - Photograph everything, from every angle, and capture every report, log, comment, observation.
